Last updated September 14, 2026

Omega Medical Records fulfills insurance requests for records
in its custody: claims review, underwriting, disability and life insurance evaluations, and audit or quality programs. Releases follow HIPAA's rules for payment and health care operations, or a signed patient authorization when one is required.

Request types we handle

Claims and payment review. Specific dates of service and record types tied to a claim.

Underwriting. Life, disability, and long-term care applications, with the applicant's signed authorization.

Audits. Risk adjustment, HEDIS, and payment-integrity reviews, scheduled in bulk where needed.

Coordination of benefits. Records needed to resolve responsibility between plans.

How the release works

1. Submit the form with the request type, the practice, and the dates of service you need.

2. We confirm the basis for release (authorization, or a permitted payment or operations purpose) and any missing paperwork.

3. Records are delivered by secure electronic transfer, with each disclosure logged.

Minimum necessary. We release the portion of the record the request requires, not the whole chart, unless the authorization asks for it.

FAQ

Questions we hear most

Can an insurer request records without the patient's authorization?

For payment and health care operations, HIPAA permits disclosures to a health plan without a separate authorization when the plan is the patient's covered plan. Life, disability, and other non-covered-entity insurers need a signed authorization from the patient. We confirm which applies on intake.

Do you handle audits and quality reviews?

Yes. Risk adjustment, HEDIS, and payment-integrity audits are fulfilled with the specific dates of service and record types requested, delivered electronically.

What is your turn around for claims?

Most complete requests are fulfilled within 10 business days. Bulk audit requests are scheduled with you so both sides know the delivery dates.
